ABSTRACT

Objective:To compare the efficacy and safety of Tonghua Dongbao′s insulin aspart injection (Rishulin) and NovoRapid (Novo Nordisk) in the treatment of diabetes.Methods:A 26-week, randomized, open-label, parallel-group, positive control drug and non-inferiority trial was conducted in 23 centers in China. A total of 563 diabetes with poor blood glucose control treated with insulin for at least 3 months before were included. The subjects were randomized(stratified block random method) into those receiving Rishulin or NovoRapid at a ratio of 3∶1. Both groups were combined with basal insulin (Lantus). The primary endpoint was the change in glycosylated hemoglobin (HbA1c) from baseline to the end of 24 weeks of treatment.Results:For full analysis set, after 24 weeks of treatment, HbA1c level of Ruishulin group decreased from (8.66±1.28)% to (7.77±1.09)% ( P<0.001), and that of NovoRapid group decreased from (8.47±1.28) % to (7.65±0.97) % ( P<0.001). Treatment difference in HbA1c (NovoRapid group-Ruishulin group) was -0.061% (95% CI -0.320-0.199). HbA1c<7.0% target reacing rates were 24.26% and 21.21% ( P=0.456), and HbA1c<6.5% target reacing rates were 9.65% and 6.82% ( P=0.310) in Ruishulin group and NovoRapid group, repectively. The standard 2 hours postprandial blood glucose (2hPG) in Ruishulin group decreased from (16.23±5.22) mmol/L to (12.65±4.57) mmol/L ( P<0.001), and 2hPG in NovoRapid group decreased from (16.13±5.37) mmol/L to (11.91)±4.21) mmol/L ( P<0.001). The fingertips blood glucose at 7-point of both groups exhibited varying degrees of reduction compared with those at baseline, repectively. Positive ratios of specific antibodies were 31.68% in Ruishulin group and 36.36% in NovoRapid group ( P=0.320). Ratios of negative to positive were 7.43% and 10.61% ( P=0.360), and ratios of positive to negative were 10.40% and 7.58% ( P=0.360) in Ruishulin group and NovoRapid group, respectively. The incidence of hypoglycemia was 60.05% and 55.40% ( P=0.371), and the incidence of adverse events was 76.60% and 77.70% ( P=0.818) in Ruishulin group and NovoRapid group, respectively. Conclusions:Rishulin is not inferior to NovoRapid, and has shown good efficacy and safety. It can be an ideal choice for clinicians in patients with poor blood glucose control with insulin.

ABSTRACT

OBJECTIVE@#To analyze the clinical features and molecular genetic etiology of a patient with 3-M (Miller McKusick Malvaux) syndrome from a consanguineous parentage family, and to explore the relationship between genotype and phenotype.@*METHODS@#After the consent of the proband's guardian and the informed consent form was signed, DNA was extracted from peripheral blood samples of the proband and her parents for chromosome microarray analysis, medical exome sequencing and parental verification.@*RESULTS@#A total of 247.1 Mb loss of heterozygosity was found in the proband with a CytoScan 750K array. Furthermore, a homozygous variant (c.458dupG) of the OBSL1 gene was found using high-throughput sequencing, which was inherited from her parents. Based on the criteria and guidelines of genetic variation of American College of Medical Genetics and Genomics, the variant is predicted to be pathogenic (PVS1+PM2+PP4), and only one case was reported previously.@*CONCLUSION@#Spina bifida occulta and lower eyelid fat pad may be a special phenotype of c.458dupG variant of the OBSL1 gene. Our study may provide a useful reference for evaluating the relationship between genotype and phenotype of 3-M syndrome type 2.

ABSTRACT

Fetal electrocardiogram signal extraction is of great significance for perinatal fetal monitoring. In order to improve the prediction accuracy of fetal electrocardiogram signal, this paper proposes a fetal electrocardiogram signal extraction method (GA-LSTM) based on genetic algorithm (GA) optimization with long and short term memory (LSTM) network. Firstly, according to the characteristics of the mixed electrocardiogram signal of the maternal abdominal wall, the global search ability of the GA is used to optimize the number of hidden layer neurons, learning rate and training times of the LSTM network, and the optimal combination of parameters is calculated to make the network topology and the mother body match the characteristics of the mixed signals of the abdominal wall. Then, the LSTM network model is constructed using the optimal network parameters obtained by the GA, and the nonlinear transformation of the maternal chest electrocardiogram signals to the abdominal wall is estimated by the GA-LSTM network. Finally, using the non-linear transformation obtained from the maternal chest electrocardiogram signal and the GA-LSTM network model, the maternal electrocardiogram signal contained in the abdominal wall signal is estimated, and the estimated maternal electrocardiogram signal is subtracted from the mixed abdominal wall signal to obtain a pure fetal electrocardiogram signal. This article uses clinical electrocardiogram signals from two databases for experimental analysis. The final results show that compared with the traditional normalized minimum mean square error (NLMS), genetic algorithm-support vector machine method (GA-SVM) and LSTM network methods, the method proposed in this paper can extract a clearer fetal electrocardiogram signal, and its accuracy, sensitivity, accuracy and overall probability have been better improved. Therefore, the method could extract relatively pure fetal electrocardiogram signals, which has certain application value for perinatal fetal health monitoring.

ABSTRACT

OBJECTIVE@#To explore the nature of chromosomal abnormality in a fetus with nasal bone dysplasia and clarify its clinical effect.@*METHODS@#Fetal chromosome karyotype was analyzed by G-banding. Single nucleotide polymorphism array (SNP-array) was used to detect the chromosomal copy number variations, and fluorescence in situ hybridization (FISH) was used to verify the result.@*RESULTS@#Fetal karyotype analysis showed an unknown chromosomal fragment in 21q21 region. SNP-array discovered a 7.5 Mb duplication in the 21q22.12q22.3 region. FISH confirmed that the unknown fragment was derived from a 21q22.12q22.3 duplication.@*CONCLUSION@#Combined use of karyotype analysis, SNP-array and FISH has clarified the nature of chromosomal abnormality in a fetus with nasal bone dysplasia, which has enabled more accurate prenatal diagnosis and genetic counseling.

ABSTRACT

Microbial biofilm, a consortium of microbial cells protected by a self-produced polymer matrix, is considered as one main cause of current bacterial drug resistance. As a new type of antimicrobial agents, antimicrobial peptides provide a new strategy for the treatment of antibiotic resistant bacteria biofilm infections. Antimicrobial peptides have shown unique advantages in preventing microbial colonization of surfaces, killing bacteria in biofilms or disrupting the mature biofilm structure. This review systemically analyzes published data in the recent 30 years to summarize the possible anti-biofilm mechanisms of antimicrobial peptides. We hope that this review can provide reference for the treatment of infectious diseases by pathogenic microbial biofilm.

ABSTRACT

Objective@#To investigate the relationship between systemic inflammatory markers such as neutrophil/lymphocyte ratio (NLR) and C-reactive protein/albumin ratio (CAR), and lymph node metastasis in patients with cN0 gastric cancer. To establish a nomogram model to predict the risk of lymph node metastasis in patients with cN0 gastric cancer.@*Methods@#The preoperative systemic inflammatory markers and clinical data of 134 patients with cN0 gastric cancer were retrospectively analyzed, and these markers of patients with negative (pN0) or positive (pN+ ) lymph node metastasis in postoperative pathological diagnosis were compared. The receiver operating characteristic (ROC) curve was used to evaluate the predictive effect of preoperative systemic inflammatory markers on lymph node metastasis. The influencing factors for lymph node metastasis were assessed by univariate analysis and multivariate logistic regression analysis. A nomogram subsequently established by R software was validated by Bootstrap resampling as internal validation.@*Results@#Compared with pN0 group, NE (P=0.022), CRP (P<0.001), NLR (P<0.001), PLR (P=0.003) and CAR (P<0.001) were higher, LY (P=0.003) and Alb (P=0.042) were lower in pN+ group. ROC curve analysis showed that the area under the curve (AUC) of postoperative pathological lymph node metastasis in patients with cN0 gastric cancer diagnosed by NLR, PLR and CAR were 0.687, 0.651 and 0.694, respectively, and the best cutoff values were 2.12, 113.59 and 0.02, respectively. The corresponding sensitivity and specificity were 62.9% and 72.2%, 77.4% and 48.6%, 74.2% and 58.3%, respectively. Univariate analysis showed that tumor size, depth of invasion, NLR, PLR and CAR were associated with lymph node metastasis in cN0 gastric cancer patients (all P<0.05). Multivariate analysis showed that depth of invasion, NLR and CAR were independent influencing factors of lymph node metastasis in patients with cN0 gastric cancer. OR were 8.084, 3.540 and 3.092, respectively (all P<0.05). The C-index of the nomogram model was 0.847 (95% CI: 0.782-0.915). The predicting calibration curve was properly fit with the ideal curve in calibration chart.@*Conclusion@#Combination of NLR and CAR to establish a nomogram model has a good consistency and can accurately predict the risk of lymph node metastasis in patients with cN0 gastric cancer.

Fluorescence in situ hybridization (FISH) and single nucleotide polymorphism-array (SNP-array) were used to analyze the component and size of the sSMCs.@*Results@#The karyotype of the fetus was determined as 47, XX, + mar[53]/48, XX, + 2 mar[31]/46, XX[14]. SNP-array has revealed four copies of chromosome 2q11.1q11.2 with a size of 2.6 Mb and three copies of 10p11.23q11.23 with a size of 20.6 Mb. The results was confirmed by FISH.@*Conclusion@#A rare chromosomal abnormality with two sSMCs was identified by combined karyotype analysis, SNP-array and FISH, which provided valuable information for prenatal diagnosis.

Objective@#To explore the genetic basis for a case of recurrent fetal congenital hydrocephalus.@*Methods@#Next-generation sequencing was carried out for the fetus, the gravida and two of her sisters.@*Results@#The fetus was found to harbor a c. 1765T>C (p.Tyr589His) mutation in exon 14 of the L1CAM gene, which was derived from the gravida.@*Conclusion@#Male fetuses with recurrent hydrocephalus should be subjected to testing of the L1CAM gene to facilitate genetic counseling and prenatal diagnosis.

ABSTRACT

Objective@#To analyze the clinicopathological features of type 2 diabetes mellitus complicated with colorectal cancer (DCRC).@*Methods@#A case-control study was conducted. Inclusion criteria: (1) hospitalized patients receiving fibrocolonoscopy; (2) adenocarcinoma and mucinous adenocarcinoma diagnosed by pathology; (3) with preoperative cTNM clinical staging; (4) colorectal cancer patients undergoing surgical treatment; (5) with postoperative pTNM staging; (6) no smoking or drinking habits. Exclusion criteria: (1) familial adenomatous polyposis (FAP); (2) Lynch syndrome; (3) carcinoma of anal canal and perianal carcinoma; (4) multiple primary cancer; (5) with serious cardiocerebrovascular diseases or multiple organ failure. Clinicopathlogical data of 32 DCRC patients who were diagnosed and treated in Peking University Shougang Hospital from December 2017 to December 2018 were retrospectively collected and analyzed. Forty nondiabetic colorectal cancer (CRC) patients during the same period were selected as control group according to the sex ratio and the age difference less than 5 years. Student′s t test and χ2 test were used to compare the difference between the two groups in baseline clinicopathological data, clinical test results, tumor markers and infiltration status of T cells in tumor immune microenvironment.@*Results@#Among 32 DCRC patients, 24 were males and 8 were females with a mean age of (63.0±1.7) years; among 40 CRC patients, 30 were males and 10 were females with a mean age of (60.5±1.6) years. The duration of diabetes mellitus in DCRC patients (from the diagnosis of diabetes mellitus to the diagnosis of colorectal cancer) was (9.2±1.3) years. The body mass index (BMI) of DCRC group was significantly higher than that of CRC group [(24.8±0.6) kg/m2 vs. (23.2±0.4) kg/m2, t=2.372, P=0.020]. There were no significant differences in other baseline data (sex, age, primary site of tumor, R0 resection rate, pathological stage, pathological type, differentiation degree of tumor, preoperative intestinal obstruction) between the two groups (all P>0.05). Serum triglyceride level in DCRC group was higher than that in CRC group [(2.1±0.2) mmol/L vs. (1.5±0.1) mmol/L, t=3.085, P=0.003], while hemoglobin [(120.3±5.2) g/L vs. (132.7±2.8) g/L, t=-2.224, P=0.029], anti- thrombin III [(94.2±3.7)% vs. (103.5±2.4)%, t=-2.197, P=0.031], and red blood cell count [(4.2±0.1)×1012/L vs. (4.5±0.1)×1012L, t=-2.055, P=0.044] were all lower than those in CRC group. The preoperative carcinoembryonic antigen (CEA) level in DCRC group was higher than that in CRC group [(50.3±21.8) μg/L vs. (5.6±1.0) μg/L, t=2.339, P=0.022]. There were no significant differences in preoperative levels of other four tumor molecular markers (CA199, CA242, CA724 and CA125) between the two groups (all P>0.05). The expression of Foxp3 [specific markers of CD4+, CD25+ regulatory T cells (Treg)] in DCRC group was higher than that in CRC group [(82.7±6.2) cell/HPF vs. (62.6±4.9) cell/HPF, t=2.586, P=0.012]. There were no significant differences in the infiltration of CD4, CD8, PD-1 and PD-L1 positive cells between two groups (all P>0.05).@*Conclusions@#The average diabetic history of DCRC patients is nearly 10 years. They have higher BMI and serum CEA level, and more Treg cell infiltration in the tumor. Close attention should be paid to these patients in clinical practice.

Objective To explore the relationship among peer relationship,self-consciousness and social anxiety in HIV-infected children. Methods Peer Relationship Scale,Piers-Harris Children's Self-con-cept Scale and Social Anxiety Scale for Children were applied to 126 HIV-infected children. Results The score of peer relationship was 53. 0(45. 0,62. 0),the score of self-concept was 52. 0(42. 0,60. 0),and the score of social anxiety was 6. 0(2. 0,9. 0). Peer relationship was positively correlated with social anxiety( r=0. 196,P=0. 029) and negatively correlated with self-consciousness(r=-0. 628,P<0. 01). Self-conscious-ness was negatively correlated with social anxiety(r=-0. 504,P<0. 01). Bootstrap analysis showed that self-consciousness played a complete mediating role between peer relationship and social anxiety,and the media-ting effect value was 0. 377. Conclusion Peer relationship,self-consciousness and social anxiety are closely related. Furthermore,self-consciousness plays a complete mediating role between peer relationship and social anxiety in HIV-infected children.

ABSTRACT

The incidence of articular cartilage defects is high.Due to limited self-repair ability,it is difficult to complete regenerative repair after injury.Platelet concentrates (PCs) is a kind of blood product obtained after centrifugation and other steps of whole blood with high concentration of platelet as the main component,which includes platelets,white blood cells,various growth factors and fibrinogen,it can promote the repair and regeneration of tissue defects and it has been widely used in orthopedics and sports medicine.PCs have been shown to play a significant role in repairing articular cartilage defects and promoting cartilage regeneration.In this paper,the PCs classification,preparation methods,mechanism of action and its animal and clinical trials in repairing articular cartilage defects are reviewed.

ABSTRACT

ObjectiveTo investigate the mechanism of HCV infection in the pathogenesis of atherosclerosis with a model of human umbilical vein endothelial cells (HUVECs) stimulated by HCV in vitro. MethodsHUVECs were stimulated with 1.0 MOI HCVcc for 48 hours. CCK8 assay was used to measure cell proliferation; flow cytometry was used to measure cell apoptosis and cell cycle; wound healing assay and monocyte-endothelial adhesion assay were used to evaluate the influence of HCV on the migration and adhesion of HUVECs; quantitative real-time PCR and Western blot were used to measure the expression of inflammatory factors and endothelial injury factors in HUVECs stimulated by HCV. The two-independent-samples t test was used for comparison between two groups; an analysis of variance was used for comparison between multiple groups, and the LSD-t test was used for further comparison between two groups. ResultsCompared with the control group, the HCV group had no significant changes in the growth, apoptosis, and cell cycle of HUVECs (all P>0.05). HCV stimulation inhibited the migration of HUVECs and enhanced their adhesion ability. Compared with the control group, the HCV group had significant increases in the mRNA and protein expression of the inflammatory factors interleukin-6 and interleukin-1β and the chemokines CXCL10 and monocyte chemotactic protein 1 (mRNA expression: t=-10.155, -12.048, -5.025, and -20.116, all P<0.05; protein expression: F=2541.739, 4806.490, 477.608, and 501.38, all P<0.001). HCV stimulation significantly upregulated the expression of the endothelial injury factors endothelin-1 and vascular endothelial growth factor and the adhesion molecules intercellular adhesion molecule-1 and vascular cell adhesion molecule-1 in HUVECs (t=-4.530, -4.497, -7.692, and -7.449, all P<0.05). ConclusionHCV can cause inflammatory changes and dysfunction in endothelial cells and thus affect the development of atherosclerosis.

ABSTRACT

Objective: To explore the influence of traditional Chinese medicine injection adjuvant chemoradiotherapy on the quality of life of cancer patients. Methods: Totally 76 cases of cancer patients were selection from PLA 307 hospital and randomly divided into the observation group and the control group with 38 ones in each group. The control group was given chemoradiation, and the observa-tion group was given Aidi injection or Shenqi Fuzheng injection intravenously additionally. The treatment course was 4 weeks. Before and after the treatment, cartesian score (KPS), simplified Chinese quality of life assessment (QLQ C30) were used to evaluate the quality of life. Results: After the treatment, the KPS was higher than that before the treatment, while QLQ C30 score decreased and was better than that in the control group (P<0. 05). Conclusion: The traditional Chinese medicine injection adjuvant chemoradio-therapy is more beneficial to the quality of life of cancer patients than chemoradiortherapy only.

Objective To investigate the effect of navigation assisted neuroendoscope hard channel technology for treating hypertensive cerebral hemorrhage in basal ganglia region.Methods Eighty-two inpatients with hypertensive cerebral hemorrhage in basal ganglia region treated in this hospital were selected as the study subjects,among them 37 cases adopted the neuroendoscope hard channel technology and 45 cases adopted the small bone window craniotomy.The operation time,intraoperative bleeding vol-ume,hematoma clearance rate,postoperative complication occurrence rate and NIHSS score at postoperative 3 months were com-pared between the two groups.Results Compared with the bone window group,the operative time in the endoscopic group was lon-ger and the hematoma clearance rate was higher,intracranial rebleeding occurrence rate was lower and the short term prognosis was better(P<0.05).The aspects of intraoperative bleeding volume and other postoperative complications had no statistically signifi-cant difference between the two groups(P> 0.05).Conclusion The navigation assisted neuroendoscope hard channel technology can improve the cure rate in the patients with hypertensive cerebral hemorrhage in basal ganglia region.

ABSTRACT

To investigate the influence of the preload and supporting stiffness on the hearing compensation performance of round window stimulation, a coupling finite model composed of a human ear, an actuator and a support was established. This model was constructed based on a complete set of micro-computed tomography (Micro-CT) images of a healthy adult's right ear by reverse engineering technology. The validity of the model was verified by comparing the model's calculated results with experimental data. Based on this model, we applied different amplitude preloads on the actuator, and changed the support's stiffness. Then, the influences of the actuator's preload and the support's stiffness were analyzed by comparing the corresponding displacements of the basilar membrane. The results show that after applying a preload on the actuator, its hearing compensation performance was increased at the middle and high frequencies, but was deteriorated at low frequencies; besides, compared with using the fascia as the actuator's support in clinical practice, utilizing the titanium alloy to fabricate the support would enhance the hearing compensation performance of the round window stimulation in the whole frequency range.

ABSTRACT

Objective To investigate the effects of CCR10 expression on proliferation,migration and invasion of NSCLC cells.Methods CCR10 siRNA was conducted into humanlung adenocarcinoma cellA549 (A549) to interfere with the normal expression of CCR10,and then to study how dose CCR10 have effects on cell proliferation,migrationand invasion in NSCLC.Results The results of MTT showed that the proliferation rate of A549 conducted withCCR10 siRNA was significantly decreased compared with Negative Control (NC) and Mock(P < 0.05).The proliferation rate of A549 in NC compared with Mock has nostatistical significance (P > 0.05).The results of Transwell cell migration assay showed that the migration rate of A549 conducted with CCR10 siRNA was significantly decreased compared with NC and Mock(P < 0.05).The proliferation rate of A549 in NC compared with Mock has nostatistical significance (P >0.05).The results of Transwell cell invasion assay showed that the invasion rate ofA549 conducted with CCR10 siRNA was significantly decreased compared with NC and Mock(P < 0.05).The invasion rate of A549 in NC compared with Mock has nostatistical significance (P >0.05).Conclusion CCR10 may contribute to cell proliferation,migration and invasion in NSCLC,and associated with the occurrence and development of NSCLC.

ABSTRACT

In recent years,with the application of natural extracellular matrix (ECM) components,tissueinduced scaffolds have become one of the hot topics in tissue engineering.As a natural source of material,ECM components are easily obtained from animals,and have good biocompatibility.Moreover,tendon ECM materials can well simulate the composition and structure of natural tendon ECM,and can provide mechanics fulcrum and stimulus signal for host cells so as to promote their tenogenic differentiation.Therefore,the tendon ECM components contained scaffold is expected to promote the repair and regeneration of tendon tissue.In this paper,the recent progress of tendon decellularization and preparation of different forms of scaffolds were reviewed,as well as the application of tendon ECM in the construction of tissue engineering tendons and mechanism of ECM induced tenogenic differentiation.Moreover,the optimized methods for preparing tendon ECM contained scaffolds were proposed to promote the application.

ABSTRACT

ObjectiveTo investigate the status of benefit finding in diabetic patients,and mediating effect of resilience between family support and benefit finding in diabetic patients.MethodsFive hundred and thirteen diabetic patients from a hospital of Harbin were recruited as investigative subjects from September 2015 to June 2016.The resilience scale,family adaptability,cohesion scale and benefit finding scale were used in the study.ResultsThe scores of benefit finding,resilience and family adaptability and cohesion were 55.67±12.87,70.65±10.39 and 104.25±15.38,separately.The total score of family adaptability and cohesion was positively related to the total score of benefit finding(r=0.389,P<0.05).The total score of resilience was positively related to the total score of benefit finding(r=0.155,P<0.05).The total score of family adaptability and cohesion was positively related to the total score of resilience(r=0.112,P<0.05).Benefit finding as the dependent variable,the results of hierarchical regression analysis indicated that family adaptability and cohesion (β=0.382,P<0.01),resilience(β=0.098,P<0.01)entered the regression equation.There was a partial mediating effect of resilience between family support and benefit finding in diabetic patients.The mediating effect was 0.011,accounted for 2.8% of the total effect.The resilience was the important protective factor for benefit finding.ConclusionFamily support has a direct influence on benefit finding and an indirect effect mediated by resilience on benefit finding.
